Before you can see one of our professionals, we’ll need to conduct an intake appointment. We can schedule this in two ways:

1. If you have Medicaid, we can come to your house and help you fill out the New Patient paperwork at your convenience. You can contact us here to schedule a time.

2. You can fill out the New Patient paperwork and either drop it off at our office, or send the forms through mail or email.

2. Fill out the form using a PDF editor such as Adobe Acrobat or fill it out by hand and scan the form.
